Leana Lopez is a Chicago-based Licensed Clinical Social Worker and consultant, specializing in clinical supervision and program design and implementation. With more than a decade of experience in the field, she has professionally supported clients in over a dozen social service organizations with focuses on behavioral health, accountable care management, youth and adults experiencing homelessness, teen motherhood, persons with developmental disabilities, addictions and recovery, child protective services, adolescent curriculum development, and art therapy. 

 

Leana graduated from the University of Chicago's School of Social Service Administration with a Masters of Social Work (AM) in 2012, and obtained her clinical license in 2018.

Program Director

Responsible for successful execution of health & wellness, job training, financial literacy, & grant programs for people recovering from addiction & experiencing homelessness. Spearheaded the development of a new strategic plan for agency, focusing on program expansion. Facilitated partnerships with residential facilities and community organizations. Supervision of program staff and management of program budget.

Director of Behavioral Health and Community Programs

Provide consultation and oversight for the development of effective care management programs for the 12 federally qualified health centers and safety net hospitals in the accountable care organization MHN ACO. Lead all behavioral health initiatives and ensure behavioral health services are integrated into care management efforts.

Program management for a professional training, job readiness, and placement program for high risk, adolescent mothers with house instability. Managed daily operation of workforce development services, including: case management services, classroom training, GED tutoring services, curriculum development, participant recruitment, and job placement. Clinical supervision of case management and training staff, budget management, and all department hiring decisions.

2011-2013

Research Assistant & Home Visitor

Member of interdisciplinary team conducting interventions for children 0-3 from targeted low-income neighborhoods. Facilitated home visits with participants. Developed curriculum and conducted formative testing for early childhood language program. Completed behavioral screenings, developmental assessments, and created module trainings for families participating in research study.

Youth Outreach Worker

Participated in street outreach for homeless youth with behavioral health needs, primarily from the LGBTQ community. Provided programming, service referrals, and resources for participants. Connected participants to primary care services at Howard Brown. Conducted informal counseling with participants. Supported youth-led leadership team.

Residential Coordinator & Case Manager

Managed group homes for people with developmental disabilities and psychiatric needs. Supervised nine direct care staff and facilitated client staffing meetings. Maintained caseload of 10 clients, wrote Individualized Progress Plans and Behavioral Programs based in Cognitive Behavioral Therapy. Worked collaboratively with psychiatrist, nurse, providers, therapists and care givers to maximize independent living and reduce behavioral health symptoms for clients. Ensured group homes were compliant with state-mandated regulations, passing HFS audits.

2007

Counselor

Curriculum development for an education enrichment program with children ages 3-11. Planned and coordinated all program activities with community partners to provide educational outings. Coordinated specialized tutoring for at-risk students.

2005-2006

Case Aid Worker

Facilitated, scheduled and supervised visits between foster children and birth parents in cases of abuse or neglect. Prepared case-notes of meetings with parents and prepared reports for use in court. Partnered with non-profit agencies and day cares in King County to provide resources for children in foster homes.
